Insurance
We accept all PPO insurances, to see if your plan is a PPO plan check the back of your insurance card or call the customer service number listed on your insurance card. If you are covered by DSHS your medical coupon has changed to a plastic ID card, please present it at all of your appointments. CHPW/Molina patient’s physician name is listed on the front of your medical ID card from CHPW/Molina.
Perscriptions
Please allow 48 hours for all prescriptions to be filled. For written prescriptions please contact our office @360-692-9362 for Silverdale clinic patients and dial 360-895-0216 for Port Orchard clinic patients. All non- written prescription refills please contact your pharmacy.
